Social Care Networks

Social Care Network (SCN) Lead Entities were established to improve integration across health, behavioral health, and social care through health-related social need (HRSN) screening and referral services. The SCNs are required to form partnerships, organize and coordinate a network, pay HRSN service providers, facilitate data-sharing, and establish a leadership team.

What are the SCNs Working to Achieve for NYS Medicaid Members?

  1. Expand access to high-quality HRSN services
  2. Enable consistent, timely screening using the Accountable Health Communities (AHC) HRSN Screening Tool and Navigation to HRSN services
  3. Create shared end-to-end visibility of the Member journey from HRSN Screening and Navigation through delivery of HRSNs services
  4. Strengthen collaboration between HRSN service providers and other partners in their regional health ecosystem, including providers, care managers, and health plans

How will SCNs work with partners under NYHER?

The New York State Department of Health Office of Health Insurance Programs maintains an SCN Operations Manual to provide program, billing, data, and data governance guidance to Social Care Network Lead Entities.

Health Care Providers play a key partnership role with the SCNs. Health care providers can contract with SCNs (and be reimbursed) to screen Medicaid Members for HRSNs, navigate Members for HRSNs, navigate Members to HRSN service providers, and/or deliver enhanced HRSN services to Members.
