NYHER Asthma Remediation Services

The New York State Children’s Asthma Initiative is proud to partner with the New York State (NYS) Asthma Control Program and Office of Health Insurance Programs at the NYS Department of Health under the New York Health Equity Reform (NYHER) 1115 Waiver Amendment.

What is the NYHER 1115 Waiver Program?

On January 9th, 2024, CMS approved a $7.5 billion 3-year package for the NYHER 1115 Waiver Amendment, effective until March 31st, 2027. This funding supports:

  • $3.5B for a program of Social Care Networks (SCNs)
  • $700M for initiatives to strengthen the health care and social care workforce
  • $3.4B for population health initiatives to improve health outcomes

NYS aims to advance health equity, reduce health disparities, and support the delivery of social care to address health-related social needs (HRSN) under this amendment through investments in Social Care Networks, Strengthening the Workforce, and Population Health.
