Home-Based Services
The New York State Children’s Asthma Initiative partners with home-based service organizations across New York State to provide evidence-based asthma home visits to children with asthma and their families. By partnering with the Lung Association’s asthma team, home-based service organizations can ensure that the care provided is in concordance with the NHLBI Asthma Management Guidelines: Focused Updates 2020 and the EPR-3 Guidelines for the Diagnosis and Management of Asthma.
Benefits of Being a Home Health Care Partner
- Technical assistance from a Lung Association partner
- Training for home health agency nurses on the provision of asthma self-management education to patients based on the National Guidelines, administration of asthma medications and use of devices, and other skills and topics as needed
- Monthly meetings with Lung Association staff, as well as yearly in-services as needed
- Resource kits for home health care nurses – including but not limited to asthma self-management education and support (Home-care Nurse Kit), asthma education flipcharts and educational materials, valved-holding chambers, pillow/mattress covers, and other trigger remediation equipment for patients as available
Home-Based Asthma Services (HBAS) Training
Taught by a Certified Asthma Educator Specialist (AE-C), this course provides guidelines-based asthma training on delivering asthma self-management education and identifying and managing environmental asthma triggers in the home. Participants receive a toolkit of educational tools and resources and a Certificate of Completion for meeting course requirements, which include a final exam with device demonstration. The HBAS training is free to participants and approximately 8 hours. Delivery can be tailored to partner needs and availability.
