Questa iniziativa quinquennale mira a migliorare i risultati sanitari legati all'asma e la qualit\u00e0 della vita dei bambini asmatici e delle loro famiglie\/caregiver che vivono nello Stato di New migliorare i risultati sanitari e la qualit\u00e0 della vita dei bambini asmatici e delle loro famiglie e a ridurre le visite di emergenza e i ricoveri ospedalieri legati all'asma tra i bambini attraverso la trasformazione delle pratiche e il miglioramento della qualit\u00e0.<\/p><p>Il progetto BREATHE NY consente ai sistemi sanitari e ai partner della comunit\u00e0 di collaborare:\u00a0<\/p><ol><li>Promuovere il miglioramento della qualit\u00e0 clinica delle cure per l'asma applicando il quadro personalizzabile del Progetto BREATHE NY con un approccio strutturato, sostenibile e a livello di sistema.<\/li><li>Educare tutti i membri del team multidisciplinare per l'assistenza all'asma per garantire un'assistenza basata sulle linee guida a tutti i pazienti affetti da asma.<\/li><li>Integrare l'educazione all'autogestione dell'asma su misura per l'individuo e i riferimenti per i servizi domiciliari per l'asma.<\/li><li>Coordinare l'assistenza tra i vari contesti e creare collegamenti con i servizi della comunit\u00e0 per sostenere i bisogni sociali del paziente e della famiglia legati alla salute o i determinanti sociali della bambini.\u00a0<\/p><p>Il progetto BREATHE NY consente ai sistemi sanitari e ai partner della comunit\u00e0 di collaborare:\u00a0<\/p><ol><li>Applicare la struttura personalizzabile del Progetto BREATHE NY per ottenere un risultato strutturato,<br \/>approccio sostenibile, a livello di sistema, per guidare il miglioramento della qualit\u00e0 clinica<\/li><li>Fornire una formazione ai fornitori per espandere la capacit\u00e0 del team multidisciplinare di cura dell'asma di incorporare le linee guida NAEPP come standard di cura per tutti i pazienti con asma.<\/li><li>Integrare un'educazione all'autogestione dell'asma completa e personalizzata e un supporto di follow-up per i pazienti, coordinato tra i vari contesti.<\/li><li>Garantire il collegamento delle cure e l'integrazione dei rinvii ai servizi basati sulla comunit\u00e0 che affrontano i bisogni sociali legati alla salute, definiti anche determinanti sociali della
