Le NYS PCAP vise \u00e0 d\u00e9velopper l'expertise des PCP en mati\u00e8re de prestation de soins de l'asthme fond\u00e9s sur des lignes directrices pour les enfants et les adultes dans l'\u00c9tat de New York. Journ\u00e9e d'observation<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

Session d'introduction :<\/strong> La journ\u00e9e d'observation du NYS PCAP commence par une session d'introduction de 30 minutes avec vous et le pr\u00e9cepteur qui vous a \u00e9t\u00e9 assign\u00e9. Le pr\u00e9cepteur fera les pr\u00e9sentations, exposera les buts et objectifs du programme et passera en revue la bo\u00eete \u00e0 outils et les documents. Vous effectuerez un pr\u00e9-test \u00e0 ce moment-l\u00e0.<\/p>

Visites de patients : <\/strong>Vous \u201csuivez\u201d le pr\u00e9cepteur pendant qu'il effectue des visites aux patients asthmatiques. En moyenne, cinq \u00e0 huit visites seront effectu\u00e9es, comprenant un m\u00e9lange de rendez-vous pour les nouveaux patients et de rendez-vous de suivi. Entre les visites, le pr\u00e9cepteur passe en revue les informations pertinentes et fait le lien entre les visites et l'asthme.

Session de cl\u00f4ture :<\/strong> La journ\u00e9e d'observation du NYS PCAP se termine par une session de conclusion de 30 \u00e0 45 minutes. Vous et le pr\u00e9cepteur vous r\u00e9unissez \u00e0 nouveau pour faire le point sur l'exp\u00e9rience d'observation, r\u00e9pondre aux questions et compl\u00e9ter un post-test. Les r\u00e9sultats des pr\u00e9-tests et des post-tests seront discut\u00e9s.<\/p>

Strat\u00e9gie de lutte contre l'asthme fond\u00e9e sur des lignes directrices :<\/strong> Vous travaillerez avec votre pr\u00e9cepteur pour s\u00e9lectionner une strat\u00e9gie de lutte contre l'asthme bas\u00e9e sur des lignes directrices que vous commencerez \u00e0 mettre en \u0153uvre dans votre pratique. Vous recevrez \u00e9galement une enqu\u00eate de suivi 6 mois apr\u00e8s la journ\u00e9e d'observation afin d'\u00e9valuer la mise en \u0153uvre de la strat\u00e9gie s\u00e9lectionn\u00e9e.<\/p>

Suivi :<\/strong> Un suivi est effectu\u00e9 avec tous les participants pour fournir un soutien suppl\u00e9mentaire sur la prestation de soins de l'asthme bas\u00e9s sur les lignes directrices et pour d\u00e9terminer l'efficacit\u00e9 du programme. Les pr\u00e9cepteurs sont disponibles pour communiquer apr\u00e8s la fin du programme et des webinaires d'assistance technique seront organis\u00e9s tous les trimestres pour tous les participants.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

Les PCP, tels que les p\u00e9diatres, les m\u00e9decins de famille et les m\u00e9decins g\u00e9n\u00e9ralistes, les infirmi\u00e8res praticiennes et les assistants m\u00e9dicaux peuvent participer.\u00a0<\/p>

Incitations pour les participants :<\/strong><\/p>

Les participants peuvent recevoir $400 pour avoir termin\u00e9 le programme ET recevoir un certificat d'ach\u00e8vement d'un programme approuv\u00e9 par le d\u00e9partement de la sant\u00e9 de l'\u00c9tat de New York (NYSDOH) et l'American Lung Association.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t
