Einf\u00fchrende Sitzung:<\/strong> Der NYS PCAP Beobachtungstag beginnt mit einer 30-min\u00fctigen Einf\u00fchrungssitzung mit Ihnen und dem Ihnen zugewiesenen Ausbilder. Der Pr\u00e4zeptor stellt sich vor, erl\u00e4utert die Ziele des Programms und geht das Toolkit und die Materialien durch. Bei dieser Gelegenheit werden Sie einen Vortest ausf\u00fcllen.<\/p>

Patientenbesuche: <\/strong>Sie \u201cbeschatten\u201d den Pr\u00e4zeptor bei der Durchf\u00fchrung von Besuchen bei Asthmapatienten. Im Durchschnitt werden f\u00fcnf bis acht Besuche durchgef\u00fchrt, darunter eine Mischung aus Terminen f\u00fcr neue Patienten und Folgetermine. Zwischen den Besuchen pr\u00fcft der Ausbilder relevante Informationen und verkn\u00fcpft Besuche und Asthma
Pflegemanagement.<\/p>

Abschlie\u00dfende Sitzung:<\/strong> Der NYS PCAP Beobachtungstag endet mit einer 30-45-min\u00fctigen Abschlussbesprechung. Sie und der Pr\u00e4zeptor kommen erneut zusammen, um die Hospitationserfahrung zu besprechen, Fragen zu beantworten und einen Nachtest auszuf\u00fcllen. Die Ergebnisse der Vor- und Nachtests werden besprochen.<\/p>

Leitliniengest\u00fctzte Asthma-Strategie:<\/strong> Gemeinsam mit Ihrem Pr\u00e4zeptor w\u00e4hlen Sie eine leitlinienbasierte Asthmastrategie aus, die Sie in Ihrer Praxis umsetzen werden. Au\u00dferdem erhalten Sie 6 Monate nach dem Beobachtungstag eine Nachbefragung, um die Umsetzung der ausgew\u00e4hlten Strategie zu bewerten.<\/p>

Nachbereitung:<\/strong> Mit allen Teilnehmern wird ein Follow-up durchgef\u00fchrt, um zus\u00e4tzliche Unterst\u00fctzung bei der leitliniengest\u00fctzten Asthmapflege zu bieten und die Wirksamkeit des Programms zu ermitteln. Die Pr\u00e4zeptoren stehen auch nach Abschluss des Programms f\u00fcr Gespr\u00e4che zur Verf\u00fcgung, und viertelj\u00e4hrlich werden f\u00fcr alle Teilnehmer Webinare zur technischen Unterst\u00fctzung abgehalten.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t

F\u00f6rderungsw\u00fcrdigkeit der Teilnehmer:<\/strong><\/p>

Teilnahmeberechtigt sind Allgemeinmediziner wie Kinder\u00e4rzte, Haus\u00e4rzte und \u00c4rzte f\u00fcr Allgemeinmedizin, Krankenschwestern und -pfleger sowie Arzthelferinnen.\u00a0<\/p>

Anreize f\u00fcr Teilnehmer:<\/strong><\/p>
