NYS PCAP aims to expand expertise among PCPs on the delivery of guidelines-based asthma care to children and adults across New York State. Meeting NYS PCAP’s four objectives will give you the foundation needed to begin delivering asthma care in your practices.

NYS PCAP Components
Observation Day
Introductory Session: NYS PCAP Observation Day starts with a 30-minute introductory session with you and your assigned preceptor. The preceptor will make introductions, outline program goals and objectives, and review the toolkit and materials. You will complete a pre-test at this time.
Patient Visits: You “shadow” the preceptor as they complete asthma patient visits. An average of five to eight visits will be completed, including a mix of new patient appointments and follow-up appointments. In between visits, the preceptor reviews relevant information and links visits and asthma
care management.
Concluding Session: NYS PCAP Observation Day ends with a 30-45 minute concluding session. You and the preceptor reconvene to debrief from the shadowing experience, answer questions, and complete a post-test. Results from the pre- and post- tests will be discussed.
Total time: Approximately 4 hours
Post-Observation Day Activities
Guidelines-Based Asthma Strategy: You will work with your preceptor to select one guidelines-based asthma strategy to begin implementing in your practice. You will also receive a follow up survey 6-months post-observation day to assess implementation of the selected strategy.
Follow-Up: Mit allen Teilnehmern wird ein Follow-up durchgeführt, um zusätzliche Unterstützung bei der leitliniengestützten Asthmapflege zu bieten und die Wirksamkeit des Programms zu ermitteln. Die Präzeptoren stehen auch nach Abschluss des Programms für Gespräche zur Verfügung, und vierteljährlich werden für alle Teilnehmer Webinare zur technischen Unterstützung abgehalten.
Participant Eligibility:
PCPs, such as pediatricians, family practice, and general practice physicians, nurse practitioners, and physician’s assistants are eligible to participate.
Participant Incentives:
Participants can receive $400 for completing the program AND receive a certificate of completion of a New York State Department of Health (NYSDOH) and American Lung Association approved program.
